To add your medical history to wisepatientAI, go to your wisepatientAI homepage on the app, and click on the "plus sign" icon found on the bottom middle of your homepage. Select "Add Medical Records" from the menu. Choose if you’d like to add your medical files from the VA, Medicare, or one of the thousands of medical providers throughout the U.S. by clicking on the provider that you’d like to add your medical records from.
Remember thatwisepatientAI is HIPAA-compliant, and you are in control of who you share your information with. Think of wisepatientAI as just the "tool" to help you access and share your medical information as needed.
You can also add medical records from multiple providers, and they will all be combined into one resource that is accessible and shareable as needed. To add records from different providers, simply follow the steps below for each medical provider.
Follow the steps below to add your medical records from one of the thousands of medical providers throughout the U.S.:
Tap the "plus sign" button. Then select the "Add Records" button. to bring up the choices of where you’d like to add your medical records from and type the name of the medical provider that you would like to add into the search bar up at the top of the access records page.
wisepatientAI gives you the ability to add your medical history from thousands of providers, including Medicare. If you don’t see your medical provider on the list, let us know and we’ll add them to the queue of providers that we reach out to.
Once you’ve found your medical provider, click on their profile and sign into their online portal to add your medical history to wisepatientAI. When you log into your provider's online account, it syncs your medical records to wisepatientAI. The final step is to click on the “allow” button to share your data with wisepatientAI.
If you don’t have an online account with your medical provider, simply create one from their website and repeat these steps.
After you’ve added your medical history, you’ll see your personalized wisepatientAI Patient Portal with your conditions, medications, and allergies. As well as physician-curated information written in easy-to-understand language that will help you better understand your health. You can continue to add additional health portals by following the same steps to log in to each portal.
Follow the steps below to add your medical records from the VA:
Go to your wisepatientAI app homepage, click on the "plus sign" button. Then select the "Add Medical Records" button to bring up the medical provider choices of where you can add your medical records from. Search for "Veterans Administration (VA) Records" choice and click on it to start the process of adding your VA records to wisepatientAI.
Click the "continue" button to allow wisepatientAI to sign in to your “va.gov” account. You will then be asked to log into your online va.gov account, which is the step that adds your medical records to wisepatientAI.
It's important to note that there are different ways that you can log into your va.gov account, and any way will work for this purpose.
Sign into your preferred option by clicking on it. Then type in your username and password and click "allow" to give wisepatientAI access to your VA medical data to finish adding your VA records to wisepatientAI.
If you don't have an account with any of these three choices, go to MyHealtheVet and click on the green “Register” button and fill out their online form.
When the MyHealtheVet registration form opens up, start by choosing your relationship to the VA by checking all of the boxes that apply.
Then fill in the information to confirm your identity to gain access to your VA records.
The next step is to choose the notifications and reminders that you'd like from MyHealtheVet.
And then add in your preference for how you'd like them to contact you, and your email address and phone number.
The next step is to choose the User ID and Password that you'd like to use for your MyHealtheVet online account.
Next, choose your two security questions and that answers that you would like to use to confirm your identity in the future.
The final step to create your MyHealtheVet online account is to check the box to accept the terms and conditions and then click on the blue "Create Your Account" button to finish creating your account.
Once you’ve created your account, follow the steps already outlined above at the start of this question/answer to add your VA medical records to wisepatientAI.
Follow the steps below to add your medical records from Medicare:
Go to your wisepatientAI app homepage and click on the "plus sign" button. Then select the "Add Medical Records" button to bring up the medical provider choices of where you can add your medical records from. Go to the "CMS Medicare records" choice and click on it to start the process of adding your Medicare records to wisepatientAI. “CMS” is simply Medicare.
Click the "continue" button to allow wisepatientAI to sign in to your “cms.gov” account, which is your online medicare.gov account for Medicare. You will then be asked to log into your online medicare.gov account, which is the step that adds your medical records to wisepatientAI.
When the Medicare.gov sign in screen pops up, type your medicare.gov username and password, tap on the green "log in" button, and click "allow" to give wisepatientAI access to your medical data to finish adding your Medicare records to wisepatientAI.
After you’ve added your medical history, you’ll see your personalized wisepatientAI Patient Portal with your conditions, medications, and allergies. As well as physician-curated information written in easy-to-understand language that will help you better understand your health.
It’s important to note that when you add your medical history from Medicare, it will pull up whatever information that Medicare has on your health.
Since it’s pulling in the information directly from Medicare, there may be a lot or a little bit of information depending on what Medicare has in your records. Please note that there is always more being added, so you can always go back to the wisepatientAI app to see what’s new. When your medical records are updated by Medicare, they are automatically updated in the wisepatientAI app.
If your records are incomplete or incorrect, please reach out to your health care provider directly to resolve those issues.
If you don’t have an electronic CMS (Medicare) account, click on the blue “create an account now” link at the top of the “CMS.gov” log-in page to create one. Below are the steps to help you create your medicare.gov account so that you can add your Medicare records to wisepatientAI.
You will then be asked to fill out the Medicare online form to create an account.
Please note that you will need to keep your red, white, and blue Medicare card handy when you fill out Medicare’s online form because you will need your Medicare number, and coverage start dates for their form.
To fill out the electronic “create account” form, add in your Medicare number, name, email, date of birth, zip code or city, and Part A coverage start date and click on the green "next" button. You will then see a security notice pop up. When you see that, click on the green "ok" button to continue creating your online medicare.gov account.
If you don’t have “Part A” coverage, click on the blue link that says, “Don’t have Part A? Switch to Part B”. You will then be able to add in your “Part B” coverage start dates instead of your Medicare “Part A” coverage dates.
You will now be asked to fill in your basic information:
- Type in your last name exactly as it is on your red, white, and blue Medicare card.
- Add in your date of birth. The month and day need to each have two digits, so if for example you were born on September 1st, then you would put in "09" for the month and "01" for the day.
- Put in your zip code.
- Then add in your email address. If you don't have an email address, simply click on the box that says "I don't have an email address" to finish filling out the form.
- The final step for this section is to click on the box to agree that you've certified that the information is true to the best of your knowledge, and then click on the green "next" button.
The last step to create your online medicare.gov account is to:
- Put in a username and password and then re-confirm your password. You'll want to write down your medicare.gov username and password because you will need them to add your Medicare records to wisepatientAI.
- Secure your account by picking a "secret question" from the dropdown menu choices and answer it.
- When you're done click on the green "create account" button to finish. You will see a pop-up message on your screen saying that you're all set once your medicare.gov account has been created.
Once you’ve created your account, follow the steps already outlined above at the start of this question/answer to add your Medicare records to wisepatientAI.
